All the information on this form is confidential.
The RMT will always inform you of what areas need to be worked on and obtain informed consent prior to assessment or treatment. Always know that you have the right to stop the assessment or treatment at any time. You are in full control.

Therapeutic massage may be performed over the sheets, clothing or with skin to skin contact and will always be attuned to your comfort level.
